Consultation Form

All clients when booking a new service such as hair colour, lashes or anything involving chemicals must have a patch test done in the salon at least 48 hours prior to their appointment.

Allergies: Clients must inform us of any known allergies prior to the appointment and alert us of any reactions at the earliest point. We have a record of all ingredients used in our products and can check upon request, but ultimately it is the clients reposnibility to tell us.

Cross contamination: if a client has any visible symptoms of illness, cold sores, open wounds, styes, anything that is contagious, you must reschedule your appointment.
